Founded in 2006, CCS began as a frontline services organization supporting Orange County California’s Indigent Program. We saw firsthand how legacy systems failed to support care teams working with high-risk, high-need populations. Over time, we evolved into a digital health company—building tools that reflect the real-world needs of care coordination.
